Universal Health Record
Parents MUST submit a complete medical form with immunization history and flu shot inoculation, and authorization for emergency treatment for each child before the start of the program. Email the completed Health Record to scott@jccbayonne.org.
Emergency Contact
Parents MUST submit an Emergency Contact and Authorization form before the start of the program.

Wrap Around Care
We offer our own extended care programs, that include early drop off and after school care, for families in the Bayonne Board of Eduction Pre-K 3 program at an additional charge.

Parent Handbook
Anyone enrolling their child in our wrap around care program MUST DOWNLOAD AND READ all of the policies and terms of enrollment stated in the Parent Handbook before proceeding to the enrollment forms below.

Credit Card Authorization
The credit card information is used for authorization when a charge will be made by the Financial Director.
