Payment options include credit card or mailed in check. If paying by credit card, please complete this authorization form. Only our Chief Financial Officer has access to this information.
Universal Health Record
Parents MUST submit a complete medical form with immunization history and flu shot inoculation, and authorization for emergency treatment for each child before the start of the program.
